I've been wondering about something concerning inquiries. Let's say I apply for a credit card, get approved, but then decide not to go through with it and return the card. My credit report will show an inquiry but (hopefully) not a new account. What concerns me is whether another lender at some later point will see an inquiry that doesn't match up with an account, and therefore will assume that I had been rejected. Does that actually happen?

No. But the account WILL most likely be reported if it has been approved.
